There are several aspects of the Asian nose that differ from a traditional Caucasian nose, which make the procedure of Asian rhinoplasty unique.  

Shape: The Asian nose tends to be less projected and flatter over the dorsum (bridge) of the nose which bothers many people who wish to have a more defined nose. The tip of the Asian nose is usually wider and bulbous and lacks definition. The nostrils tend to be more prone to flaring and the base of the nose is often too wide and out of proportion to the rest of the face.  

Skin: The skin of the Asian nose tends to be thicker and more sebaceous. This thickened skin can cause the tip of the nose to appear more bulbous or boxy and the skin itself more prone to scarring or scar contracture at the incision site.  

Cartilage: The nasal cartilage is the intrinsic framework that shapes the outward appearance of the nose. Asian cartilage tends to be less abundant and softer than in other ethnicities, thus causing the tip of the nose to be less defined and under-projected.   

Bones: The nasal bones tend to sit more obliquely, which causes a wide nasal vault that is de-projected off the face. This can contribute to breathing problems, which may be improved by rhinoplasty. 

Surgical Technique

Nose surgery is very personalized and tailored to your needs and goals. You will first consult with Dr. Costa to determine your goals and desires before deciding on the appropriate technique. Dr. Costa will use 3D imaging technology to simulate a natural result that you can expect to achieve with surgery. The surgery will be performed at an outpatient surgery center under general anesthesia. Asian rhinoplasty will often require an open technique, which is a tiny incision that is hidden on the undersurface of the columella to access the underlying structures of the nose. 

Often due to a lack of cartilage, other sources for cartilage may be needed. Dr. Costa prefers to use your tissue and will discuss what options will provide enough support for your nose. Cartilage may be harvested from your rib or ear to gain enough strength to structure the nose to the desired shape. Cartilage grafts can also be used from other sources to rebuild the nose and increase projection along the bridge and tip. In patients with a wide nasal base, a small incision hidden in the groove between the nose and the cheek (alar groove) is made in order to narrow the base of the nose. Dr. Costa does not use any foreign objects for implants or grafts in the nose, such as silicone or Gortex. These implants can puncture the skin over time or shift and deform the nose. In rare instances, they become infected and can have devastating consequences. Although popular in other countries, many of these implants will need to be revised or removed over time.  

What to Expect

 This procedure will be performed using general anesthesia as an outpatient treatment, so you can return home the same day after being cleared by Dr. Costa. After your procedure, you should go home to rest. You may have a splint on the outside of your nose or in your nostril in order to hold the shape of the nose while it heals. Rhinoplasty is not considered to be a very painful procedure by most patients, but you should expect some bruising, and discomfort for the first few days as you begin to heal. You will see back Dr. Costa within one week of surgery to remove your stitches and any splints on your nose. At this time, your breathing should be immediately improved and will continue to get better with time. Each case will be different in terms of results and how long it will take to reach your final outcome, but generally speaking, about 75% of the swelling will be gone after four months from surgery. The last 25% of swelling can take anywhere from 1 – 2 years to completely resolve to see your final outcome, but you can enjoy your newly shaped nose for years to come. This procedure can also be combined with a chin augmentation or injectable fillers to improve results and create more facial harmony.
